Emergency Services Training Notification Form

Departments that wish to open their training drills, hosted classes, or other courses to county public safety departments, stakeholders or departments from adjoining counties should complete this form. Information will be used to advertise the course on the website, send out training alerts, and can also be used to take course registrations at the host department's request. Provide an overview of the class and training objectives.
Select the agency/department hosting/facilitating the class.
Indicate the start and estimated end time for the course.
Include location address.
List any items students need to bring (e.g., SCBA, Turnout gear, Duty weapon/holster/equipment, traffic vests, BSI, textbook, paper, etc.)
Select all that apply.
If there is a student cost, provide that information in the student equipment/materials section above.
If YES, an EMA staff member will contact you soon to coordiante the registration process.
Identify the abosulte deadline for accepting student registrations.
