For a copy of your NGB22, NGB23, or other records, complete and sign Standard Form 180. The completed form can be emailed to ng.ia.iaarng.mbx.g1@mail.mil, faxed to 515-252-4139, or mailed to

Standard Form 180 can also be completed in our office during normal business hours.
